SETMA is a moderate sized multi-specialty, private medical practice which started using electronic health records in 1998. Early on, the electronic health records expanded to support electronic patient management, utilizing chronic disease management tools to improve the care provided while addressing patient safety and cost of care issues. SETMA tracks over 200 quality metrics with each patient treated, audits provider performance over panels and populations of patients and analyzes data with business intelligence tools to find leverage points for improving care. SETMA also publicly reports by provider name the performance on these metrics at Public Reporting. The first four
steps of SETMA's Model of Care are utilized in the design and execution of Continuous Quality Improvement within the practice. As a NCQA Tier II I and AAAHCMedical Home, SETMA currently focuses on Care Transitions, Medication Reconciliation, ethnic disparities and decreasing preventable readmissions to the hospital.
